When do you know to change the grease in the wheel bearings?

Matter of personal taste.
They will go on nearly forever, but it isn't a bad idea to pull the hubs, completely disassemble and clean the bearing for inspection at fifty or sixty thousand mile intervals. Just slamming grease in the outter bearing is a waste of time and might lull you into complacency.
Good quality bearings properly packed with good grease and adjusted properly have an infinite life expectancy under normal use.

Also, if you've acquired the car and maintenance is unknown - do it. Clean bearings and examine carefully.
On wire wheels, if you need to change out rotors, might as well - do it. Replace inner seal as well.
